| What You May Expect With Each Type of Investigation |
AOE / COE / WORK PLACE INJURY CLAIM STATEMENTS: Typically includes tape-recorded interviews with the Claimant (unrepresented), the Employer, Supervisors and Witnesses. During the Claimant interview information is obtained regarding his / her usual duties with the Insured and the events leading to the filing of the claim. Other information obtained / examined relate to the Claimant's prior history of employment, injury / medical history, as well as prior claims filed. The Claimant interview is extremely detailed and usually takes one to two hours, sometimes longer in stress / psychological claims of injury. We obtain a signed and dated medical authorization form from the Claimant during his / her interview and make pertinent confidential notes regarding all injury-related details such as the Claimant's gait or demeanor during the interview as well as relevant living conditions. Video Surveillance (Aka: Sub-Rosa) investigations: Typically include eight hours or more of on-site video-recorded observations. Most investigations typically begin at the Claimant's residence and may migrate to other areas as may be dictated by the Claimant's mobility. Hidden cameras are routinely utilized during non-vehicular surveillance and such video is in color and of the highest quality. The client is provided with such a thorough report that review of the entire video tape, at the conclusion of the assignment, is not necessary. Obtained video tape is consistently steady and focused with the proper framing of the Claimant. An initial close-up identification shot of the Claimant is routinely obtained whenever possible. Privacy issues are always considered during any Sub-Rosa detail and the Claimant's privacy rights are never violated in any manner during these covert assignments. STATEMENTS Statement-taking typically involves meeting with the subject of the investigation or witness and obtaining a detailed digitally tape-recorded description of events relating to the topic of investigation. Such statements may relate to death claims and resulting dependency issues, claims of discrimination or alleged improper employer treatment following the filing of a claim, apportionment or subrogation issues as well as serious & willful allegations. Other reasons for obtaining thorough statements from individuals may relate to mortgage insurance inquiries, criminal or civil actions as well as auto theft investigations for the insurance company.
